Has she used this IELTS certificate in any of her prior application ? If yes then she can again use it without any problem.

If not, then if this IELTS certificate is still valid (not expired) and is in the list of Home Office's English Qualifications, then she can use it.

Are you applying on the basis of Long Residence or Tier 2 ?

If Long Residence then write 9 years 4 months and if Tier 2 then please write 4 years 11 months.

The extensions for Tier 1 are closed after April 2015 and not 2018. After April 2018, there will be no more ILR for Tier 1 but I am not sure what would happen in your case.

Dependents who applied for their visa before 9th July 2012 only need 2 years to qualify for ILR. Since your wife came in 2010, should would have qualified with you in 2014. Better late than never .. so apply for her ILR now
