You can submit your application

Puma,

Anytime after 1 year of getting your ILR i.e 30th April 2014. Just because you applied in Sep 2012 is not going to exempt you from the requirement of 1year of being free from Immigration Restrictions.

You need to first switch to FLR(M) and then board ILR. IMHO, Hence it will make sense to send separate envelopes. Separate forms preferable

They are notorious for misplacing stuff. Separate tracked applications are always better. You can see the link for the 28 day rule.

You can apply on completion of 9 years and 11 months.(Generally 28 days prior to the 10yr limit).

Apply straight away. They are notorious for bringing in rule changes. And now their decisions cannot be questioned.

You can do a deed pool

Before you apply for your British Passport , get the British Passport in your new name and then surrender your Old Indian Passport, get surrender Certificate and then apply for your OCI with deed poll . There should not be any issues.

Put the date when you first landed in UK

If you landed at heathrow put London, If you arrived at Edinburgh mention that.

Where your stay has not been continuous since you first arrived and if you have applied for BC after your second arrival, you can put the date you arrived the second time and continued to get your ccitizenship.
