I entered on oct 2010,but had to leave uk .
But again left and came in April 2011.

Yes ,as long as you fulfill the visa requirement,it's fine for prior extension

From all the post that I have read ,one cannot stay more than 180 days outside UK in 12 months period.

I guess it's not fair for people who got tier 1 in 2011 ,since they might need 2 nd extension,and because of this sudden rule change.what are their options?

I think you need to apply for 2 nd extension,
Since you arrived in UK for the first time beyond 90 day bracket.
