Which form for FLR: O or HSM?

For those people who recently applied/are applying for FLR under HSMP, what is the correct form?

Is it the new FLR 0, which came out for 04/2005 or is it the old FLR (HSM) from 08/2003? Or is it something else?

Just to clarify...

The only thing I showed the immigration official was my passport with the EC visa stamp. I also said that I was arriving for the first time on HSMP.

I had all of the other documentation, but I was never asked to present them, and I didn't show any of it to the official.

Question about NHS eligibility

When are HSMP visa holders eligible for medical treatment under the National Health Service (NHS)?

I think I remember reading somewhere that it was after 2 months of living in the UK, but I would like to be sure. Does anyone have an official link to this information?

Does anyone have a courier service that they would highly recommend, especially for an international delivery?

This is to get documents back from HSMP, not to send stuff to them. Sending is usually pretty simple.

If so, did that letter mention that they were currently working on June 2004 applications?

If not, how did you get your reference number (03601X)?
