1.17 - no do not write down your current address as you've already written this in the previous section

2.4/2.5 - yes, definitely. This is a new application and a new form so just ignore whatever you've submitted in the past and fill everything in that they ask you to again.

From my experience the booklet does not provide detailed information and that is why you need to do further research on the information provided by UKBA on their site and not be selective as to which you choose to follow. Yeah, technically you can fill out the entire referee section for them as long as it's their signature at the bottom. So it wouldn't be a problem that you're filling in their past address.

Online printout is fine. I have a Pay As You Earn (PAYE) arrangement , so is NI number sufficient ? Just fill in your employer's details in section 3.1-3.5 and of course fill in your national insurance number in 1.10, you don't need a statement of account.
