I got an ILR as a PBS applicant

She was pbs tier 1 dependent Her visa was received in May
2013 but she arrived here in September 2013 then in January tier 1 was about to expire so she got a pbs dependent leave to remain till 2016.
She will be completing one year as a dependent in September probably then..

My wife has recently acquired an pbs dependent Leave to Remain ,valid till 2016.I have An ILR.

I have to apply for Pbs visa,in my passport both my First name and Last name are under the first name field.how should i fill in my name in the paper based form here? Will this cause a problem?
