If payment is by cheque , what all needs to be filled

If I am making the payment by cheque, what details needs to be filled in the first page ?
Will the debit card number , expiry date should also be filled ?
Will the account holders signature is required ?
Since the cheque is having the signature, shall I leave the account holder signature blank in the form
