5 yrs continuous period from date of appearing @ PEO ?

Hi
I along with my family is completing on 12/June/2014 as Tier 1 (General).
I can book an appointment at PEO any day after 15 May 2014 (because it is within the 28 days from the date of completing 5 years continuous period).

I shall appreciate to know if the continuous period is worked out from the date for personal appearance at PEO OR from 12/June/2014.

If you are Completing 5 years on 12th June 2014 than Yes you can apply on or after 16th May at PEO
If your application successful you will be granted ILR from the date of your Personal Appearance at PEO.
In simple words Qualifying Period for ILR is ''4 Years 11 Months and 2 days''.

Is there any rule for ''4 Years 11 Months and 2 days''? Further how I am going to work out the 5 years continuous period either from 12 June 2014 or from the date of my appointment @PEO because it will be different in both the cases.

Yes Anyone who have Tier 1 General Visa need to spend legally 5 years in UK and they can apply prior 28 days before completing 5 years.
if you deduct 28 days from 5 years its comes up 4 years 11 months and 2 days Understood :|.
I assume as per your query You initially granted Tier 1 General as on 13th June 2009 and as on 12th June 2014 you are completing 5 years.
So in this way you are eligible either to apply by post or in Person in between 16th May to 12th June Straightforward.

Is there any link on the UKBA home page which will provide me support that I along with my dependents are eligible to apply withing 28 days of the completion of 5 years continuous stay.

Page 3, point 5 'when to apply' of the Set(O) guidance and also if you search the forum and look at other ILR success stories, you will see the detail provided to you are accurate.

5 WHEN TO APPLY
You and any dependants applying with you should
apply before the end of your/their permitted stay in
the UK.
There is a qualifying period to complete in most of
the categories on this form. It runs either from
- the date on which you entered the UK with a
visa
in the relevant category; or, if you did not enter
the
UK with such a visa, from
- the date on which you were first granted permis-
sion to remain in the UK in the relevant category.
If you entered the UK with a visa several weeks or
more after the date from which it was valid for use,
you may need to apply for an extension of stay to
complete the relevant qualifying period.
Please do not apply more than 28 days before
completing the qualifying period.
If you apply
earlier than that, your application may be refused.
If that happens, we will not refund the fee and you
will have to pay again when reapplying.
The qualifying periods are as follows:
5 years In the work permit holder, employment
not requiring a work permit, businessperson,
innovator, investor, self-employed lawyer, writer,
composer or artist, UK ancestry, Tier 1 and Tier 2
categories or routes.
