ILR for Tier1 General and extension for dependant

Hello All,

I am currently on Tier 1 General and will be completing 5 years (on Tier1 G) in a few days.
My wife is on Tier1 G dependant visa from the past 1 year.

I am now planning to apply for ILR and my wife for extension of visa since she has completed only 1 year.

Which application form should I and my wife use?

We are also planning for PEO appointment. Can we jointly apply via PEO appointment at the same time.
