Tier 1 and dependant extension at the same time

Hi,
My Tier 1 expiry date is on next month (20th July 2014). I am planning to submit my application this week. I am submitting the application for my wife too at she same time.
Can someone please confirm me whether I should fill and submit two separate form for main applicant and Dependent.
I am referring to the following forms

Many told me that I can submit just the main applicant form as the dependent details are added there.
I am confused please help

Whoever said that you can only send the Main applicant form as the dependent details are mentioned is absolutely WRONG.

You have to fill a separate FORM for each dependent you are applying for.
