dependent visa as a doctor

Hi everyone, I am a doctor from india. My finance was on general tier 1 visa and just got his ilr. So if I apply for dependent visa, I ll get the same. How ever, since I am a doctor, I need to study further and practice there. But in the guidelines it is written that a person cannot work or train as a doctor. So how should I apply to go around this rule? Please reply it is urgent.

P.s- I am writing plab examination to work under gmc which all international medical graduates have to take .

I mean as your spouse is settled you will be applying for spouse leave, thus you can work without restrictions including training as a Dr. A1 English and Financial Requirement, form is VAF4A and Appendix 2.
