Dear Ms/Mr <insert caseworker surname>
Today I received my Certificate of Application in the form of your letter to me, which is reproduced in whole and enclosed with this letter. I believe some of the information contained there (as highlighted) is incorrect. I hereby request that a corrected Certificate of Application be issued to me which contains corrections addressing the following issues.
It is clear that I have the right to work in the UK, by benefit of my spousal relationship to an EEA Citizen, <insert EEA's name>, who is relocated to the UK for purposes of exercising EU Treaty Rights under the applicable EU Directive as amended. Further, I have provided indisputable evidence of my spousal relationship with the EEA citizen in the form of our marriage certificate along with proof of our identifications in the form of my spouse's <insert EEA passport type (eg Irish)> passport plus my original <insert non EU passport type (eg USA)> passport.
I look forward to receiving a revised Certificate of Application just as soon as possible as it is my intention to work and provide for my family immediately upon recognition by
the Home Office of my right to do so. Thank you for your timely consideration of these facts.
