ILR Declaration

Hi. I am about to send my wife application off for ilr. with regards to her signing the set m document her passport is in her maiden name when she entered this country she has ben using our married name with all her correspondence on the married name.

does she sign the set m on her passport signature or her married name signature? if anyone could reply back to me asap as I am due to be sending off her application on Thursday many thanks
