eea4 application need Help filling forms

Hello dear members!
was filling up EEA4 and got puzzled on few questions..

a bit of background first: I'm in UK since January 2004, came on Student visa... met my ,now wife, and overstayed visa.. has been caught but not deported, because of family. my obligation was: monthly signing at Immigration enforcement office. However, got legally married and entered UK in 2008 as spouse of EEA member, applied for residence card an received it in Sept 2009...

so what do I mention in application on following questions:
1.how long you lived in UK?

2.any civil judgements or penalties under UK immigration act?
3.when you arrived to UK

Just confused if its 5 year qualifying period or the whole time?

Any kind of help kindly appreciated!

is it just 5 qualifying years we are talking about in EEA4?

entered UK 2 times: as a student first, and as sea family member... which one counts?

Hi I apply for Eea4 it's over 6 month but I didn't hear any thing from home office I don't no what to do
