EEA family Member New Entry clearance from UK embassy Pak

I am Pakistani national has been issued a 5 years UK residence document form UKBA London for a family member of EEA national. I came to Pakistan. My nephews dismantled some of the pages of the passport. I went to British High commission Islamabad. My passport has been kept by British High Commission Islamabad. The British High Commission Islamabad has advised me by letter dated 03 December 2014 that I should apply for new entry clearance if i wish to travel to the UK. Please advise ME which visa category i should apply for UK entry clearance so that i could join my EEA spouse living in the UK.
