ILR form for depandants -when applying together

Hi All,

I am applying for ILR as a tier 1 (general) migrant along with Spouse and child (6 years).

Do I need to fill in separate form for Spouse and form for a child?

Sorry to ask but during Tier1 extension (July12), I had to fill in separate forms for each of us..

can some one please confirm?

I called UKBA and asked they said its a single form :)
hope thats correct :)
