Spouse Visa with two kids - one application form?

Please can someone advise to apply for a spouse visa from abroad is there one form enough for spouse and two kids under 16? or they need to fill separate form for each.

Also what about appendix 2 form, is one form enough?

One form is not enough. Each applicant should have their own form - You making three applications here.
The same applies to Appendix 2 form, each applicant should have their own.
