New EEA3 application form

Hello.
I am looking for new EEA3 permanent residency application form and I found only form from 06/2014 and something from 30.1.2015.
I am confused because that first one has only around 40 pages and second 121 pages.
Which one I should download?

I think u have to use now new version of the form which is valid from 30.01.2015. This is bit lengthy but may be not all section relevant to u

Hello all,
I have some questioned regarding the new PR form

is it mandatory for one to print and include all in one's application as the new eea3 form is bulky and sometimes when the Evidential document proof required is mentioned says at least 2 of the listed; is it requesting for 2 of each or any 2 out of the listed documents required.

In application as question when i first time arrived to UK. does it mean first time first time??
I came first time in 2007 just for 2 months but i am applying 5 years from 2010. Which date I should put there?
