I just booked my PEO appointment, and paid the fees online. (woops, didn't realize that now you have to pay the full fee at the time of booking the appointment!!). Now, how do I fill the first section in the form, that pertains to the fee?

you don't have to fill the payments page as you have paid the fees on-line and make sure u take the print out of on-line payment with Ur application .
