Passport surrender query

Hi
I have following query regarding passport surrender form

1. what should I put in place of issue (British Passport section)
2. what is the citizenship aquired date for minor? data on the certificate or the date when concil issued registration certificate
3. Can I send one postal order or bank draft for 3 applications?
