Tier 1 Dependent Visa Form (Outside UK)

Hi All

I need to apply Tier 1 G dependent visa for my dependents from Pakistan. I was filing online application form but come across few questions that i'm not sure what to answer. please help me

1) How long do you intend to stay in the UK?
Is it "3 Years" because this visa will be valid of 3 years (as mine)?

2) What is your permanent residential address and contact details?
Now my wife lived with me in UK from last 2 years. Buy since she is applying from pakistan should she give Pakistan address?

3) Have you ever received medical treatment in the UK?
My wife lived in UK so she has use nhs and medical facilities. (She is not suppose to pay for it because she is Tier 1 G Dependent ) Should we mention it

4) Please enter the CoS/CAS number (main Applicant)
They have asked for Main applicant CoS/CAS number. What is that?
