ILR Application in April 2015

Hi Guys,
I am new to this group and need small information. Pls. help me out.

1. I came in the UK on Tier 1 In 2010 - May 11th and booked the appointment for same day service on 18th April 2015. Is it ok and its 28 days before?

2. My wife came here in the UK on 9th June 2012 though she got the tier 1 dependent visa on 9th May 2012.

Quesitons- Which form I and my wife have to use for our ILR application next month?
We both have passed the Life in the UK test in 2012. would that be still valid for our ILR application?

I am working as a contrator and have my own limited company setup. May i know the docs needed?

Yes LIUK pass is still valid.

Use SET (O) form, just one form for both of you, mention your wife as dependent in your application. For the list of docs you can search the forum as lots of postings mention about the documents that can be used.

Do you mean that I need to fill only one form with my wife details under dependent section ? or seperated form for both SET O?

Someon confused with that my wife needs to fill SET M.

One Set(O) form for both of you, with your wife as dependent of you as main applicant. If you look at the form you will see the relevant section for her as dependent.

She cannot use Set(M) as she does not have the qualifying visa as spouse of person with ILR or a British citizen.

One of my friend just went with his wife with different SET O forms thats why I got confused.

Also, Do I need to carry my limited company returns as well?
