Hi Amber Thanks for your response. She went to India for family reason. While she was in India our address was same, Joint bank account was there, I used to send her money to support her through local Money exchange operator, I bought her return ticket from my Credit card, also before she travelled we checked with few people and they said she can stay outside uk less than 180 days for a 12 month period. As your 1st visa granted on 22 Oct 2010 and your 5 years will be complete as on 22 Oct 2015 . As per rules you can apply 28 days before to complete 5 years .
28 days before to 22 Oct 2015 will be 24th September 2015 So you are eligible to apply ILR on or after 24th September 2015.

relevant documents in 12A, you must provide the following documents:
Evidence specified in section 9A of this form
For the period before you were granted leave as a Tier 1 (General) migrant, documents
showing that you met the relevant requirements of the immigration rules.

This question is not for Tier 1 General applicant leave it blank just tick 1st one.
