Immigration Health Care Surcharge-ilr

Do i have to pay this surcharge when applying ILR in 2016 ??
what is this surchage all about , doesnt NIN goes to this ??

It doesn't apply to ILR applications.
(Casa, not CR001)

Thank you , so my wife will extend her pbs dependent visa on april 2016 , how much surcharge has to pay ?.
