Extension of Tier 2 and NHS surcharge

Hello,

I would be applying for my renewal of Tier 2 (general) visa in June within UK. My employer would be process the CoS only on 18th May. I would like to know
1) how long does this process take ? Should I receieve my CoS by mid June ?
2) Once I post the application, how long will it take to get my passport back ?
3) My current contract has been renewed for 16 months. On UKVI website, when I attempt to register for the NHS surcharge, it asks for £600, shouldn't I be paying ~£300 only ? If I pay £600, will I get the remaining amount back ?
