Which application form to use

Hi

I have achieved my ILR based on PBS tier2. My wife is dependent on me. She is still on tier 2 dependent visa. For further extension, she needs to use which form "Application for a Grant of Leave and Biometric Immigration Document as a Tier 1, 2 o r 5 Dependant" or "FLR(m)"?


She has completed 5 years. She has ENGLISH NARIC comparability and ESL. But she does not have life in the UK.

Please tell me which form to use?
