Tier2 General Extension Time + Driving Licence Points

Hi Guys

At the moment I am on Tier2 (General) Visa.

1) I am going to extend the Tier 2 (General) Visa for another 3 years.
The current Tier 2 (General) Visa be expired at the end of the June 2015. Can I make an application in the middle of May 2015 via Premium Service? Isn't it too soon?
Is it alright to make an appointment 45 days before the expiry date? I did not see any time limit for making application in advance?

2) Should the points on the driving licence be mentioned in the application form?

you can make extension application whenever u want, even a year before the expiry date if you want to.
