What all documents I have to send with AN form

What all documents I have to send with AN form

Hello good people

I would like to know one more things. I read from this https://www.gov.uk/government/uploads/s ... h_2015.pdf

on the end is everything what I need to send togerther with the form but I am not sure what everything they want.
What I think all what I need to send them is only the AN Form, Life in uk certification, english certification and passport, that's all, right?
I have not Parntert, I am in EEA, I meet all requirements.

I do individual method and just for me. Do you know if I need to send some another documents please?

Do you meet residence requirement? What documents are you sending to prove it?

This is what i am going to send, is there something more what I need to send?

What residence requirement? I filled in the form my days out of UK for the last 5 - 6 years.

Have you lived in the UK for 6 years (5 years exercising Treaty right + 1 year after PR)? Do you have payslips, P60s, employement letters and bills to prove it? CSI if you were a student?
• Evidence of exercising Treaty Rights for 5 years for the relevant EEA national. This may include:
• Evidence of funds and comprehensive sickness insurance for self-sufficient persons and students, for example:
▪ a valid European Health Insurance Card (EHIC) issued by a Member State other than the UK; or
▪ a valid S1, S2 or S3 form; or
▪ a comprehensive private medical insurance policy which covers the holder for treatment in the UK in the majority of circumstances.
• Evidence of state pension (not occupational pension) for retired person.
• Medical report if permanently incapacitated
• Evidence of five years continuous residence in the UK
• If applying as the direct family member of an EEA national, evidence of relationship to that EEA national.

So I put also for example those P60 covering 5 years to envelope + life in uk + english certificate + passport and I shuld be ok, right? Thanks for helping me.
