ILR and BRP name change

Hi,
My wife is in ILR since August 2014. She changed the surname to my name in November 2014 and, we were due for a baby that time.

Now she is outside UK. How do we change the name in BRP now? do we need to pay any fine as the situation was different..

can she travel with the same BRP to UK? without changing name...Please let me know your thoughts..
