affidavit for explaining name difference in documents

Hi

I am having a problem related to name differences in certificates. I had read about this in different threads.

My name is mentioned in my degree certificates as "Anand K". But in my passport/Driving License/pay slips, the name is mentioned as "Anand Kishore".

I believe there will be a problem for my HSMP application due to this.

Will this be resolved if I submit an affidavit?
Please provide me a format for the affidavit.
Also, should it be done on stamp paper?

I went to a notary public and he said that the affidavit can be filed like:
I write a letter stating that I am known by the names X, Y & Z and I sign it. The notary would sign below stating that he knows me and I have signed this statement in front of him and the signature is mine.
Is this the way the affidavit should be filed?

Even I had the same problem, and I submitted an affidavit stating that the above mentioned names belong to me, the same person.
and notarized by local registrar.
