Tier 2 (General) Extension query

Hi I am just about to attend a Tier 2 premium service appt on 20/6/15. Have already paid £600 IHS. Just want to check my application will be OK if that is possible as its a lot of money to waste!

I arrived in the UK on a Tier 2 (General) visa on 5/11/2011 (valid from 27/10/2011 - 30/09/2012)
I extended my Tier 2 (General) leave and was granted from 02/07/2012 - 15/08/2015
I am now applied to extend again the T2G.
I am changing my employer and starting a new job on 17/08/2015 (and leaving my old employment on 04/08/2015)
My Job decription is also changing from Trainee General Practitioner (GP) to GP (after completing 3yrs training).
My present salary is £51,000 and new job will be £75,000.
COS was assigned on 11/6/2015.
Is this application going to be OK?
Would really appreciate if so can confirm before I attend the Premium centre.
