Help Needed with ILR - Tier 1 VISA Urgent

Hi

My status is as follows

Date of Tier 1 VISA Issued - 5th Jan 2011
Date Entered in UK - 16th Sep 2011 (Did not have a UK Job)
Date Left UK - 4th November 2011
Date Entered the UK Again - 19th Dec 2011 - (Had a UK Job)
Date Tier 1 VISA First Renewed - 23rd Dec 2012
Date Tier 1 VISA 2nd Renewal - 23rd Jan 2015
Tier 1 Visa Valid until - 22nd jan 2018

My question is as follows

1. Am I eligble for ILR ( I have spent more that 180 days each year only spent a maximum of 2 months in a year out of UK on vacations)

2. Date when I would be eligible for ILR

3. Do I have to satisfy the criteria for ILR (similar to Tier 1 renewal points) only for the last 12 months or for all the 5 years?

4. What documentary evidence is needed for me and my wife and child.

Sorry for the long list of queries. Would appreciate any help in this matter.

Can you list out your absences(exact dates) ?

If its not more than 180 days in a year then you will be eligible for ILR on 28 days before 16th Sep 2016 provided you meet all other requirements
