Countersignature question help needed please

Dear members and seniors

Please can any one help me with below questions in Section 10 'Countersignature' of passport application form? These may sound silly...

1) I confirm the I have known the person names in sections 2........

Do we need to write name of applicant or only in case of child applicant?

2) for (insert years) as (please say how........)

we need to write years which makes sense, but do we need to write 'as colleague' or just 'colleague' in spaces next to yrs?

I wish I could attach image of the section but forum exceeded quota I guess..

My countersignatory lives far so I have to post him the form and photo, any mistakes will waste lot of time. Someone who have recently applied for first British passport please advise.

We kept it simple...

In our case:

1) Yes we wrote in the name (even for an adult's application).

2)
03 <-- for number of years
&
Teacher <-- for type of relationship/profession

As long as it is understandable I doubt you will be penalised for adding or omitting an "as"
