Tier 2 General Migrant extension Premium Service

Dear Members,

My wife is currently on a Tier 2 General migrant visa which expires soon. She has secured an unrestricted COS for a in-country extension. For the amount of time left , she will have to go for a premium service urgently.

I have 3 queries :
(1. ) Does she have to fill up the online form first? OR, whether she should first book an appointment and pay the fees online to secure a premium service appointment?
(2. ) Which form does she need to fill up for a Tier 2 General migrant extension through Premium service? Is it the paper form https://www.gov.uk/government/uploads/s ... _04_15.pdf
OR is it the UKVI tier 2 online application form
OR both?

Any guidance would be REALLY helpful as we are confused between filling up the paper form and the online form

(3.) The paper form begins with asking card details of the applicant. Yet, one would have paid the full fees online while making the online Premium service application. Therefore how does answer these questions on the paper form relating to card details?

what do u mean the amount of time which is left, she has to go apply for premium application?
she can always apply for a normal application as well, considering she posts her application before the expiry of her visa.
