Got HSMP approved - Help on applying for FLR(HSMP)

Hi All,

I have got my HSMP approved and now I have to apply for FLR. I have been looking in these forums for a covering letter to send along with my FLR, but couldn't find any. I would be grateful if someone could post a covering letter format here.

Also, I am planning to submit the below documents. Can someone confirm if these documents are enough.

1. FLR(HSMP) form.
2. My Passport & 2 Photos.
3. My dependant Passport and 2 photos.
4. Marriage certificate.
5. Last 3 months payslips.

Any help would be highly appreciated.
