missed adding countries visited in Visa application form

Hello All,

I missed adding 2 countries i visited as a tourist in Visa application online form under the category "Countries visited other than UK"

But i have added 2 countries which i last visited for official purpose.

Two countries which i missed to add, their Visa is stamped on my OLD passport
Two countries which i added, their Visa is stamped on my New passport

On monday i have Visa appointment scheduled. Please let me know if this will lead to refusal?

It's not a problem as long as you submitted your old passport that has VISA stamped.
Some people leaves it blank and mention that refer to attached passport.

Yes i will be submitting my old passport also.

Just wanted to know if i should inform the embassy about this and manually correct it?
or should leave it like that?

As long as you are giving correct information in your visa application, you are fine.
Forgetting to mention about visiting different countries is not a problem as those info can be found in your passport. I don't think you need to inform this to UKBA. This is completely my opinion. You can wait for expert advice.
