How many applications when applying with a dependent

Hey guys,
Sorry if this is very obvious, but I could not find a direct answers in this forum and also in the internet.

If I am applying for ILR through Tier 1 route (first entry on 1 Nov 2010) for me and my dependent at the same time, do we apply in one SET(O) form or two separate forms (one each)?
