Tier 2 unrestricted COS and TB test issue

Many thanks for the support till now.
I have two specific queries.

My employer is going to issue me an unrestricted COS and I am applying along with my wife as dependent.

First question:
I asked the employer, they said the process of unrestricted cos issuance is also similar (like it needs to be authorized by home office once request to issue unrestricted cos is sent). I want to ask is that the case or is unrestricted cos issued on the spot as soon as the employer fills in the issuance form for this cos.

Second question:
About TB Test, my wife came to UK in April as my dependant and is in UK, will she still need to supply a new TB certificate (she had already done same when she applied for entry clearenace in April).

Third question:
Address proof: does my wife need to submit any uk address proof with applicaton?
