Page 13 of the HSMP Application Form

Dear Friends and Gurus in the house.

I need your advice about filling page 7 & 13 of the HSMP Application form. I am submitting my e-tax certificate not covering the whole period claimed except for Nov and Dec 06. I have claimed exceptional Consideration because i have other docs as requested. I am claiming from Nov. 2006 to Oct. 2007 as my period under consideration. I have tax certificate for just Nov. and Dec 2006 because tax year runs from Jan to Dec.

I dont know if I should tick Income Tax Cert as part of evidences supplied.
I did'nt tick it under my earnings because it did'nt cover the whole period claimed.


Should I tick it as part of my evidences on Page 13 and Page 7 of the form?

Please advice.

No, you shouldn't because your tax certificate and salary slips will not corroborate for the full claimed period. As you said, you are ticking exceptional consideration because your tax certificates are not enough to cover the full year you are claiming.
