NHS health charges

Just a general question - for those who don't need a 3 year extension .. and are only going for a 3rd extension to cover a few months remaining to qualify for ILR. Do they also have to dish out GBP 600 ?? Or they have some choice to just request an extension for 1 year?

What category visa extension is it?
Char (CR001 not Casa)

PBS Dependant - she will be short of 4 months to be elligble for ILR and hence will need a 3rd extension.
