Form AN Employment Section Help required

Hi,

I am self-employed and director of three companies very confused about following on Form:

FORM AN PAGE 10
1.46 Employee , BUSINESS PARTNER , SELF EMPLOYED , A DIRECTOR ( Which one I tick)

1.47 Employer : I HAVE THREE COMPANIES WHICH COMPANY NAME NEED TO PUT.

1.48 Employer Address : Which company address need to provide.

For Q1.46 why not tick more than 1 box.

Suggest explain your full circumstances (all the companies & etc) in the 'additional info' section of the form.
