Regarding Name Change Document

Post by shaaskul » Fri Nov 30, 2007 7:22 pm

Hi Paul,

Thanks for the reply and the blog link. I got good insight about the proof of assets. I also have one more query regarding name change.

Actually my name was changed after my college. So my name in all the certificates from college/university will be different till my first offer letter. After that all the documents would bear my new name including my passport.

I have the TamilNadu Gazette publication in which my name change was published. But it is like a booklet in which all the name changes during that period got published. Please advice me - How to proceed on that. What I am thinking is I can write a cover letter indicating my name change and enclose the gazette publication document giving reference to the page number and the section.

Thanks,
S

Hello Shaaskul,

You have to go to your nearest notary in your city.
They will have a process to issue you an affidavit.
Make a request there.
Get an affidavit stating that your name1, name2...(list all names) belongs to the same person Shaaskul's-Full-name-as-in-passport.
Attach this affidavit along with your application.
Explain this in your covering letter.

If the name change is only an expansion-of-initials-in-passport, you will not be needing this affidavit.

If you find my blog helpful for you, please pass that information to your friends.

Thanks
Paul
