Applying for ILR from Tier 1 G- query about form

Hi,
I am applying for ILR with a dependant minor child. I have the following questions:
1) Do I need to fill in 2 Set-O application forms- one for me and one for my minor dependant?
2) Do I need any cohabitation documents for my minor dependant? He has extended Tier 1 General visa twice before as my dependant.
