Dependent Visa

Hello all,

I am in UK under the HSMP scheme and residence permit stamped in my passport until 2009 nov. Now I would like to apply for a dependent visa for my Wife, she is in india. Could anybody help how to proceed this like which application form to use, guidance notes, Documentations, where to apply etc.
