Concern on Sponsor Note

In got Cos with following details
Start date : 15 March 2016
End Date : 14 Feb 2019
Salary : 22,500
Now HR amended COS with following NOTE
NOTE: Start date of role is now 15 April, and has received annual increment to salary and is now 23,000.
Does this note make any problem?
Also what start and end date I have to use in my visa application and for IHS pay ?
I am about to apply asap.

Have you made an application to the Home Office to remain in the UK in the last 10 years?
Do i have to mention Entry clearance application details or just LTR application details?
