New Tier2 application fees

Hello,

I am going to apply for a new Tier2 application (via premium service) to move to another company. I have the following questions:
-At what point do I pay the visa application fees online? Is it after or before I book the appointment?
-Is there any way to cancel the appointment later on, and get a full refund for the visa fees?
