Surrendering India Passport - Queries

I will be visiting VFS to surrender Indian passport. There are few points which are not clear on VFS website.

1) On form (http://www.vfsglobal.com/India/UK/pdf/A ... 271115.pdf) It is written PLACE AND DATE towards the end of form. Will it be LONDON AND DATE or WOKING AND DATE (As I live in WORKING) or will it be UK AND DATE

2) I am visiting alone and also will be carrying forms to surrender passport for my wife and son. Will they allow to submit application for family? or do I need to carry some sort of authorisation (Again this information is not on the website)

3) To get the surrender certificate, I will be using courier. Do I still need to provide self addressed envelope or courier will provide that?

Will appreciate your help on this.

I would use Woking and date ( actually it does not matter at all )

Normally they dont ask for the close family members , ( I had carried with me and the format is there in the website on Indina visa application page )

You don't have to provide envelope, if you use their courier service ( it is only 50 p difference for this service if you use DX courrier vs postal Nest day delivery for upto 500 gms)

Will appreciate your help on this.
