Visa for newborn baby in Uk

I am working under tier 2 general and my baby born in UK 3 months back and i am planning to apply tier 2 general dependant visa from inside UK.

can you please anyone provide the below information.

is it need to fill form in Online? if yes then please share website link.
do we need to pay IHS for child?
what are the documents are need to attach with application.
