Tier 1 Entrepreneur Dependent switch to Tier 2 ?

Hi, my wife is a Tier 1 Entrepreneur Dependant (PBS Dependant) & her company is happy to sponsor her Tier 2 visa.
1) How does she have to prove English language efficiency? She has done her Engineering in India.
2) Will she get a restricted COS or Unrestricted COS? what's the difference?
3) Does she have to go to India (country of origin) to switch category?
4) Can a Tier 2 Dependent run a company without restriction?
