Hello All,

I am trying to apply for HSMP in 2 weeks time. I am facing a problem as
explained below.
All suggestions would be really helpful and appreciated.

My full name is "Kannan Alagesan".
As per the usual practice in South India, in some of my documents a short
name "Kannan A." is given.

My Details:

Nationality: Indian
Current Status: Working in France on a work permit.
Documents with full name: Passport, US Driving License (I worked in US for
few years)
Documents with short name: India driving license, under-graduate &
post-graduate degree certificates & mark sheets
Document for age proof: US Driving License or Indian Driving License (I
don't have any birth certificate)

My questions:
1) Can I apply for HSMP with a Driving License issued by USA as the age
proof though I hold an Indian passport and stay in France currently?
2) If I get 'initial expansion certificates' from the universities where I
did UG and PG, will it help to overcome the problem of short name? If so,
what all should be mentioned in the certificates?

Hi,

I had applied with the name.initial format [Degree certificate and driving license. I did not provide any explanation for it.

I got my HSMP approved.

I didn't know what evidence to provide and i was in a hurry so i took the risk.

Do you have all the documents in the Name.Initial format?.
Few of my documents are in the Name.Initial format(my degree certificate, mark sheets etc) and few are in the firstname surname format( my passport, pay slips etc). In this case what do you think is advisable?

Even most my certificates except passport are in Name and Initial format only. I explained that in my covering letter of HSMP application. Got approval also... so might not be an issue, if u still want to provide some proof then I think better to get notary letter stating both the names represent the same person.

Hi,

I am planning to apply in two weeks, could anyone please send me the format of letter from notary for name (initial) not expanded in degree certificate or the covering letter format to be submitted?
